This form allows applicants who are experiencing difficulties submitting an application by their program's deadline because of a
widespread disruption,
which affects access to your means of completing or submitting an application
requirement
. In case of such a disruption, we may be able to provide you assistance with submitting your application without one or more required application materials, although
it does not waive the requirement
. You will still be required to submit the missing item as soon as circumstances allow.
The information you provide will be visible to the admissions committee, who can choose take it into account when reviewing your application. Depending on the missing item(s), it is possible that your file may not be considered complete enough for review.
Please note that processing of this request by Graduate Admissions, including temporary waiver of a requirement to allow you to submit your application, does not imply that the requirement(s) will be waived for the purposes of admission review or decisions. Missing material(s) may be viewed negatively by the committee or prevent the review of your application.
Guidelines:
1. You must have started an application to a graduate program before submitting this form.
Your request will not be processed without a valid application reference number. Additionally, requests will not be processed without a detailed explanation of the widespread disruption affecting your ability to provide the application requirement.
2. This form may not be used to request application fee waivers:
Most academic units have a process for requesting a fee waiver due to financial need. Do not use this form to request a fee waiver. If you cannot locate the appropriate place on your application to request a fee waiver, please contact us at gradadmissions@uchicago.edu. We will not process or respond to inquiries about fee waivers via this form.
3. If the material is not required or you qualify for a waiver of the requirement, we will not process the request.

We do not approve hardship requests that are related to personal or family circumstances.
Only hardships connected to a natural disaster or other widespread disruption of services, such as access to the internet or electricity, or affecting schools and testing centers' ability to operate and provide services such as transcripts or score reports, qualify for assistance with submission. Although we understand that the costs associated with applying to graduate school can be significant, we encourage you to plan for these costs, as well as the costs associated with attending a graduate program, and to read your program's information on application fee waiver availability carefully.
